MAKES REVISIONS TO HCFA-484.2 FORM

The Office of Management and Budget has approved the use of the revision of the Oxygen Certification of Medical Necessity (CMN). The effective date for the Program Memorandum (B-00-08) is April 1, 2000. The changes to the CMN are as follows:
The disclosure statement on the back now states that the estimate for the time required to complete this information has changed from 15 minutes to 10 minutes.
The address for suggestions to change forms now reads "HCFA, 7500 Security Blvd., N2-14-26, Baltimore, Maryland 21244-1850 and to the Office of the Information and Regulatory Affairs, Office of Management and Budget, Washington, D.C. 20503."
All verbiage to the instructions on the back page has been changed from "ordering" physician to "treating" physician.
The date of the form has been changed from 5/97 to 11/99.
The DMERCs will continue to accept the CMN dated 5/97 (DMERC 484.2) with claims received on or before September 30, 2000. The revised form (DMERC 484.3) is required for claims received by the DMERC on October 1, 2000 or after.
It is expected that the DMERC will provide suppliers with the revised HCFA Form 484 (11/99) in the spring or summer Supplier Manual updates.
In order to avoid denials on claims submitted on or after October 1, 2000 providers should implement the new form ASAP. Work with software vendors to assure you are able to transmit the new form number with the electronic CMN record.
